Africa is increasingly adopting artificial intelligence to bolster overstretched health systems. The Gates Foundation and OpenAI have launched a $50 million initiative to support primary healthcare delivery across the continent. The programme, named Horizon1000, aims to deploy AI tools in frontline clinics. These tools will help solve chronic staff shortages. They will also improve the quality of care.
The initiative was announced at the World Economic Forum in Davos on Wednesday. It will begin in Rwanda. Over the next two years, it will expand to other African countries. The Gates Foundation says Horizon1000 aims to reach 1,000 primary healthcare clinics and their surrounding communities by 2028.
Africa turns to AI as health worker shortages deepen
Sub-Saharan Africa faces an estimated shortfall of nearly six million healthcare workers. Experts say this gap cannot be closed through training alone in the near term. The World Health Organization has warned that workforce shortages are undermining access to essential care across the region. This is due to underinvestment and the migration of skilled professionals.
Bill Gates said artificial intelligence could play a critical supporting role in low-income settings. In these settings, demand for care far exceeds available human resources. Speaking in Davos, the Microsoft co-founder argued that AI is particularly valuable on the “delivery side” of healthcare. It helps clinicians manage workloads. AI organises resources and reduces administrative burdens.
Under Horizon1000, AI tools will be used to support tasks like patient intake, triage, and clinical decision support. They will also assist in follow-ups and referrals. Additionally, they will provide access to trusted medical information in local languages. The partners have stressed that the technology is intended to support health workers, not replace them.
Rwanda chosen as pilot country

Rwanda has been selected as the pilot country for the initiative. This choice reflects its reputation as a regional technology and innovation hub. The government has actively promoted digital solutions in public services. These innovations include healthcare. Recently, it launched an AI-powered Health Intelligence Centre in Kigali.
Rwanda’s Minister of Information and Communications Technology and Innovation, Paula Ingabire, welcomed the partnership. She said AI-based decision support tools aid more than 60,000 community health workers operating across the country. She said the technology help improve consistency and quality of primary care delivered at community level.
Horizon1000 is expected to expand into other large African health systems, starting from Rwanda. Kenya, South Africa, and Nigeria are among the countries cited by Gates as potential next phases.
Aid pressures push new approaches
The rollout of the AI health initiative comes amid growing concern over declining international aid budgets. Gates has warned that recent reductions in global development assistance have had a severe impact. These reductions have contributed to the first rise in preventable child deaths this century. This situation is increasing pressure on already fragile health systems.
Organisers say the Horizon1000 programme is designed to help African countries do more with limited resources. It aims to improve efficiency and decision-making. The programme focuses on enhancing strategies rather than substituting traditional funding models. Critics, though, caution that AI deployments must solve data quality. They must also tackle infrastructure gaps. Ensuring privacy protections is essential. Additionally, addressing the risk of algorithmic bias is crucial.
Despite these concerns, proponents argue that carefully implemented AI systems could significantly improve access to care. This improvement is particularly relevant in rural and underserved communities where clinician shortages are most acute.
A growing role for AI in African health systems
AI has been used in isolated health projects across Africa for years, including disease surveillance, diagnostic support and logistics planning. Analysts say that Horizon1000’s distinguishing factor is its scale of funding. It focuses on integrating AI into everyday primary care. This contrasts with small pilot programmes.
OpenAI chief executive Sam Altman said developers have a responsibility. AI systems must deliver real social benefit. This is particularly important in healthcare. He described the partnership with the Gates Foundation as an opportunity. It will test how advanced models can improve outcomes in real-world, resource-constrained settings.
